Brain contusions commonly occur following motor vehicle accidents, falls, and other forms of blunt-force trauma. Although CT and MRI are essential tools for identifying focal structural injury, they provide a limited view of the underlying neurological damage.

Traumatic impact initiates a complex cascade of secondary injury processes that extend beyond the contused region itself. These include:

  •  Neuroinflammation, driven by microglial activation and cytokine release, impairs synaptic signaling and contributes to prolonged cognitive and behavioral symptoms.

  •  Metabolic dysfunction, resulting from mitochondrial impairment and disrupted cerebral energy balance, leads to reduced processing speed, mental fatigue, and diminished cognitive endurance.

  • Disruption of neural networks, particularly within white matter tracts, where shearing forces and secondary degeneration interfere with communication between brain regions critical for attention, memory, and executive function.

  • Microvascular injury and blood–brain barrier disruption, which alters cerebral perfusion and perpetuates inflammatory responses, further exacerbating neuronal dysfunction.

Importantly, these secondary mechanisms can evolve and do not necessarily correlate with the size, location, or apparent severity of the contusion seen on imaging. As a result, patients may experience significant and persistent neurological symptoms despite “stable” or “unremarkable” radiographic findings. Accurate interpretation, therefore, requires expert neurological analysis that integrates clinical history with appropriate evaluation and longitudinal assessment.
